On July 1, the Badminton Asia Confederation and the Indonesian Badminton Association issued an announcement stating that during the mixed team group match of the 2024 Asian Youth Badminton Championships held in Indonesia, 17-year-old Zhang Zhijie of the Chinese badminton team suddenly fainted and died after being sent to the hospital.

The young badminton player died young, which is a pity. The Chinese Badminton Association’s official website posted a message expressing deep condolences for Zhang Zhijie’s unfortunate death on the court. The Indonesian Badminton Association and the local hospital held a press conference and said that Zhang Zhijie died of cardiac arrest.

There are many cases of sudden cardiac arrest in sports competitions. At the European Cup three years ago, Eriksson suddenly fell to the ground without any confrontation and suffered a cardiac arrest. Fortunately, with timely rescue, Eriksson recovered and returned to the court this year.

On July 3, in response to the above incident, Wei Meng, director of the general internal medicine department and director of the heart center of Shanghai Jiahui International Hospital, said that sudden death caused by various heart disease reasons is cardiac death. This type of sudden death often occurs suddenly and progresses rapidly. The heart activity with hemodynamic collapse suddenly stops. Symptoms often occur within 1 hour from the onset to death. It is usually caused by persistent ventricular tachycardia or ventricular fibrillation.

“Early studies have shown that there are about 550,000 sudden cardiac deaths in my country every year. There are significantly more men than women, and nearly half of the sudden deaths occur in people aged 40-50,” said Wei Meng.

How should we rescue this kind of sudden death? Zhang Lu, deputy director of the emergency department of Shanghai Jiahui International Hospital, said that the golden time for rescuing sudden death is 4 minutes. As the rescue time is delayed, the chance of rescue will drop significantly. After 4 minutes, the brain is often irreversibly damaged. Once a sudden death is discovered, bystanders with certain first aid knowledge should immediately carry out high-quality cardiopulmonary resuscitation until professionals arrive to provide subsequent advanced life support.

Zhang Lu revealed a statistic that the success rate of treating sudden death caused by cardiac arrest outside the hospital is only about 1%. The number of sudden deaths in my country is as high as 550,000 each year. Even in the United States and Japan, where AEDs (automated external defibrillators) are more widely used, the success rate of rescue outside the hospital is less than 10%. “Let more people master effective cardiopulmonary resuscitation methods, so as to gain precious opportunities for patients to be rescued before professional medical personnel arrive.”

How to prevent sudden cardiac death?

Wei Meng pointed out that regular screening is necessary first. Currently, electrocardiograms can be used to check for genetic ion channel diseases such as preexcitation syndrome and Brugada syndrome, as well as severe atrioventricular block. If there are chest pain symptoms, myocardial ischemia and acute myocardial infarction can also be checked, and echocardiography can screen for the presence of cardiomyopathy. Stress tests such as treadmill exercise tests, cardiopulmonary exercise tests, and coronary CTA can screen for congenital coronary vascular abnormalities and existing severe coronary atherosclerotic stenosis in advance, and are suitable for young people who have exercise needs or have had previous exercise-related cardiac events.

“For existing family history of sudden death, existing family members should be recommended to undergo relevant gene screening. Currently, clear pathogenic genes have been found for some hereditary cardiomyopathies and ion channel diseases,” said Wei Meng.

Wei Meng also pointed out that in addition to screening methods, it is also important to improve lifestyle habits and control cardiovascular risk factors, which requires the following:

First, a low-salt, low-fat diet can reduce the intake of trans fatty acids and saturated fatty acids, and reduce the intake of sugar, which can reduce the occurrence of hypertension, dyslipidemia, and diabetes.

Second, for those who already have hypertension, diabetes, and dyslipidemia, they should take medication regularly, monitor blood sugar and blood lipids regularly, and home blood pressure measurement is encouraged.

Third, quit smoking and drink less, no more than 2 standard cups a day.

Fourth, reasonable and regular physical exercise, such as walking for half an hour at least 5 days a week, can improve cardiopulmonary endurance. Physical exercise should be avoided when having heart symptoms.

Fifth, ensure a reasonable sleep time. Sleep time and heart time have a U-shaped relationship. Generally, 6-8 hours is more reasonable. Too much or too little is not suitable. Do not stay up late for a long time to avoid biological clock disorders.

Sixth, you need to pay attention to mental health, learn to manage your emotions, avoid long-term exposure to mental stress, actively seek ways to vent, and seek psychological counseling and drug intervention from professionals when necessary.
